I'm pretty sure this is a map of "pre-flood" Middle-Earth; you'll notice that on the upper-right of the globe is labeled "Beleriand". It is slightly annoying that it won't go any bigger, but on the same continent as "Beleriand" is a line that is labeled "Ered Luin". Those are the Blue Mountains (I think - correct me if this is not right) that are West of the Shire.
I checked, and yes, the Blue Mountains are the Ered Luin. So, in answer to your question, LOTR took place in the rather extreme North-East of this globe. It's interesting, because this shows a lot of things that are hard to conceive otherwise -like how Valinor connects to Beleriand - which is how a group of Elves (including Galadriel, IIRC) got from Valinor to Beleriand. It also shows exactly how much land was covered in the big flood thingy that happened when the Numenoreans tried to sail to Valinor. Veeeryy interesting.

Yes - the parts we're most familiar with are just east of the Blue Mountains. But their shapes are different.
For perhaps a better-rendered version, see The Atlas of Middle Earth by Karen Fonstad. Also - Midge - not quite. Numenor doesn't exist yet in this one. The surface of the world was remade somewhat significantly from the end of the First Age to the start of the Second. In fact - much of Second Age Middle Earth looks a whole lot like what is left in the Third Age. Again - Fonstad's Atlas is a nice reference.

Yes - after the end of the First Age is when Beleriand was sunk beneath the sea, and Numenor was raised up for the Three Houses of the Edain.
