Well if you are talking about his last meal in Hobbiton, in Bag End, then it was just Frodo and Pippin with Sam, it was tea time, and Lobelia was there with Lotho but they were not offered any, do they count? But if 'familiar lands' are just the Shire in general then his last meal was in Crickhollow with Merry, Pippin, Frodo, and Fatty.

...sigh you are so close. The answer (specific) is in the book. It is derived by knowing the characters involved and what lands were indeed "familiar", in this case to Sam. I'll give you until Monday before I give up.
