Officially a world wonder does only include a man-made structure so not a whole city or something alike.
Thus the great world wonders of Middle-Earth should be:

1Khazad-Dum:the great hall of a thousand pillars (hall 21 I thouht it was called)
2The statues of the Argonath.
3The pillar of reflection at the harbor of Umbar. *
4The bridge of Khazad-Dum.*
5The great lighttowers of Eryn-Beraid in which a palantir was placed.
6The tower of Amon-Sul, also with a palantir.*
7The fort of Helmsdiep, the Hornburg.
*These wonders had been distroyed at the end of the Third age.

I didn`t include any of the great towers like the Orthanc, Barad-Dur, or the Minas-Tirith or Minas-Morgul because they are sort of a city of there own.
Older wonders (the wonders of the first age) could be the thousand caves (Menegroth), Gondolin, Vilyamar, Nargothrond, Nogrod, Belegost and the Thangorodrim. although off course these are also cities.
