Reading LOTR

When I read lotr I pronounce all names and places as if they where English. Does anyone else have difficulty in pronouncing them or are attempting to pronounce them just as Tolkien had in mind?

OK, this might seem a bit silly.
When you read lotr, do you read it out aloud or do read it in your mind, so to speak?
